Denver Health Social Worker CSW (HCM) in Denver, Colorado

Job Summary Under general professional supervision, performs social work/case management services including planning, organizing, treatment and inpatient discharge/disposition. Delivers social services in accordance with the approved plan of treatment and plan of care. Assists the physician and other healthcare members in understanding significant social and emotional factors related to the patient’s health problems. This position works overnights Saturday and Sunday in the Emergency Department from 8pm-6:30am Minimum Qualifications Education: Master’s Degree in Social Work from a CSWE accredited college or university.

Experience: Typically, one year of experience.

Knowledge, Skills & Abilities: Knowledge of emotional and social factors that effect patient’s health. Knowledge of discharge and in-service education programs. Excellent interpersonal skills. Skilled in identifying problems/opportunities to improve care. Ability to help the patient and family to understand, accept, and follow medical recommendations. Ability to utilize resources, such as family or community agencies, to assist the patient resume life in the community. Ability to collect key information from the patient, family, and the community, as part of the social work assessment. Ability to communicate to the appropriate staff, information about the patient’s discharge plan. Facility: Denver Health Main Campus, 777 Bannock St.
