Denver Health Social Worker MSW in Denver, Colorado

Job Summary Under general professional supervision, performs social work/case management services including planning, organizing, treatment and inpatient discharge/disposition. Delivers social services in accordance with the approved plan of treatment and plan of care. Assists the physician and other healthcare members in understanding significant social and emotional factors related to the patient’s health problems.

Minimum Qualifications Education: Master’s Degree in Social Work from a CSWE accredited college or university.

Experience: Typically, one year of experience.

Knowledge, Skills & Abilities: Knowledge of emotional and social factors that effect patient’s health. Knowledge of discharge and in-service education programs. Excellent interpersonal skills. Skilled problems/opportunities to improve care. Ability to help the patient and family to understand, accept and follow medical recommendations. Ability to utilize resources, such as family and community agencies to assist the patient to resume life in the community. Ability to collect key information from the patient, family and the community as part of the social work assessment. Ability to communicate to the appropriate staff information about the patient’s discharge plan.
