Community Options Supported Living Services Assistant in Montrose, Colorado

PURPOSE:To provide assistance and training to adults with developmental disabilitiesin order to help them achieve more meaningful and productive lives inaccordance with the values statements of the organization.JOB TITLE:Supported Living Services AssistantDUTIES:1. Provide services and supports as identified in each assigned person sIndividual Service Plan, such as: respite, personal care, homemakerservices, including cooking and cleaning, community integration, medicalappointments and other activities. This may include Behavior Support Services.2. Interact with all individuals and all staff at all times with appropriatevoice tone, level and pitch, language used, gestures and physicalmovements in compliance with individual rights while at all times respectingthe rights of individuals and staff.3. Function as a professional support to provide formal and informal trainingand assistance in activities of daily living: dressing, bathing,eating, cooking, menu planning, cleaning, etc.4. Function as a mentor to provide formal and informal training and assistancewith social interaction skills: talking, writing, leisure activities,treatment toward self and others, etc.5. Supervise the administration of medications according to state of Coloradoapproved guidelines.6. Use agency vehicles to transport individuals to and from communityoutings/activities and scheduled appointments as required.7. Attend work as scheduled which includes mandatory training sessions,staff meetings and meetings with managers.8. Comply with all duties, policies, procedures, protocols and all otherwritten or verbal directives.9. Provide supervision and support to individuals in community settings. 10.Accurately and legibly complete all skill training, progress notes,personal care, medication administration records and other work relateddocuments. Participate in the creation and writing of developmental SP goalsand quarterlies.11. Accurately and legibly complete all time sheets, leave requests,accident/incident reports, etc. on time.12. May include taking on call phone for SLS transport and transporting asneeded.13. Observe clients for signs of illness or changes in health.14. May be required to work weekends and holidays.15. Work with minimal supervision and recognize situations where assistance isnecessary.16. Promote and maintain appropriate professional and ethical relationships inaccordance with policies, rules and regulations.17. Satisfactorily complete other duties as assigned.REQUIREMENTS:-High School diploma, GED, OR 1.5 years relevant experience-Valid CO driver's license with acceptable record (for agency insurancecompany)-Acceptable criminal background check-Successful completion of Medication Administration Course-Must be able to meet these physical demands/work in the followingenvironmental conditions: -move clients in/out of beds, wheelchairs, vehicles, and commodes -occasionally lift and/or move up to 50 pounds alone and up to 100 poundswith assistance -exert up to 50 pounds of force occasionally and 20 pounds of force often -walking, sitting, stooping, kneeling, crouching, using hands/fingers -using close, distance, and peripheral vision -using spoken words; listening and perceiving the nature of sounds -may be subject to chemicals and infectious diseases (trained on andrequired to follow universal precautionsPREFERRED SKILLS:-Human service experience preferred, but not requiredWAGE:$9.30 + per hour, DOEBENEFITS:Health Insurance AvailableDAYS/HOURS:Full-time position with varying schedule.APPLICATION INSTRUCTIONS:Click "how to apply" for instructions on how to apply for this position.Applicants must be registered with and can do so atno cost.
