Rite of Passage is looking for a Case Management Assistant to join it'sStudent Services team at Ridge View Youth Services located in Watkins, CO(South Aurora area).The Case Management Assistant is primarily responsible for overseeing andensuring all students receive the best level of care and Placing Agenciesreceive the highest level of service delivery in the areas of requireddocumentation, communication and general logistics and planning.ESSENTIAL FUNCTIONS: Tracks and routes Type I and Type II incident reports. Maintains a student master control roster; updates the student rosterwith the site physical counts. Maintains the student movement logbook and facilitates the site database. Coordinates the monthly case file audit. Assists with the completion of the intake/admissions process for newstudents. Collects, collates and disseminates admissions documentation,develops and constructs the initial student file and manages and maintains allstudent files. Communicates effectively with site staff and outside agencies and providesstudent information to all appropriate departments. Acts as the primaryliaison with outside agencies, if applicable. Coordinates and hosts Probation Officer visits. Assists the designated supervisor with various tasks including but notlimited to: phone calls, greeting guests, communicating with andassisting staff members, other departments and outside agencies, orderingapproved office supplies and program necessities, copying, filing, etc. Assists in editing and disseminating Case Management reports Documents and disseminates the Treatment Committee, C&P meetingsagendas, minutes etc. Records exits, intakes and daily count tracking for reporting purposesincluded in the KPIs. Develops the Coordination and Planning Calendar (C&P) weekly. Works with the Clinical and Medical departments to secure authorizationsfor medication changes. Signs and distributes Service Authorization Forms (SAF) Completes required reports and documentation in a timely manner, providesmanagement with required reports and advises of any problematic situations. Provides encouragement, guidance and resources to staff and students whenneeded.MINIMUM QUALIFICATIONS: High School diploma or equivalent. Two years of experience in a related field preferred. Ability to understand, support and knowledgeably report on the ROPProgram, Positive Peer Culture and Student progress. Must be an eligible ROP driver. Ability to prepare written reports and correspondence, to effectivelycommunicate, verbally/in writing, and follow verbal/written instructions. Must have excellent organization and time management skills; flexible andable to handle multiple priorities, adjust to high pressure and rapidlychanging business conditions. Ability to provide exemplary customer service to all employees and outsideconstituents. Ability to function independently and as a member of a team in a multi-taskenvironment.
