Some flexibility possible per needs of people receiving services.LAST DATE TO APPLY: Posting will close when candidate has been selected for hire.General SummaryThe Resource Coordinator is assigned a caseload composed of children withdevelopmental disabilities. Resource Coordination functions may include:development and coordination of an Individualized Service Plan, serviceaccess, case documentation, and monitoring of supports and servicesadvocacy on behalf of persons with disabilities.Essential Functions1. Possesses strong verbal and written language skills in both English andSpanish; translates documents and communications as needed2. Schedules, facilitates the development and prepares the IndividualizedFamily Service Plan and Child Outcomes Summary in accordance with departmentprocedure3. Ensures ongoing service provision and documentation of needs throughsix-month reviews, transition plans, and annual planning4. Gains funding approval for services through completion of accurate,detailed requests submitted to the Director of Support Services5. Distributes information regarding children s needs to secure timelyservices per department procedures6. Completes the Child Outcomes Summary Form at entry and exit fromearly intervention7. Maintains solid understanding and ability to communicate informationrelated early intervention services, including the funding hierarchy8. Monitors services through review of monthly progress notes and transitionreports9. Assist in the transfer of case management services to appropriateagencies when notified of a move10. Provides ongoing advocacy to the child and their family to assure rightsas are respected.11. Schedules, facilitates and documents meetings to, including but notlimited to, address concerns, changes in need, and service plan revisions12. Maintains ongoing monthly contact with children s families as well asface-to-face contact every six months to monitor overall satisfaction,determine appropriateness of ongoing service goals, and to address anyconcerns if applicable. Performs, documents in the RC Contact NoteDatabase, and bills for an allowable case management activity monthly asoutlined in the 7/1/09 Quick Guides to TCM Services and to Quality Log Notes13. Ensures referral for placement on Family Support Services Program orappropriate waiver waitlists by the child s 3rd birthday as appropriateOther Functions1. Maintains Master Record and ensures accurate data entry in CCMS, EIProvider database, and RC contact note database as needed consistent withdepartment process and standards2. Adheres to the concept of confidentiality related to staff members,consumers, families, and service providers3. Works cooperatively with other community agencies involved with thechild, such as, department of social services, mental health,Tri-County health, and school districts4. Meets department expectations for response to all correspondence within24 hours of receipt to include voicemail, email, and requests for information5. Participate in committees as need in accordance with the duties of theposition6. Participate in departmental meetings, training, and shared tasks asrequired7. Participate in activities required of employees of North Metro.8. Drive as needed in performance of the duties above.9.
